Patients saved money by choosing in-network facilities regardless of the site of care, whereas insurers saved by increasing the usage of [ambulatory surgery centers (ASCs)] for common adult outpatient surgeries paid by commercial insurers. Insurance models that better align patient and insurer incentives could increase utilization of ASCs and lower overall spending on outpatient surgeries.

Although surgical checklists and other safety protocols have been introduced across various surgical fields, preventable adverse events still occur, highlighting the need for additional research. This cohort study found that ß-lactam prophylaxis was associated with significantly lower [surgical site infection] rates than non–ß-lactam agents, and a reported ß-lactam allergy did not independently predict infection risk. Implementation of an allergy-focused stewardship guideline was associated with a marked reduction in non–ß-lactam use, underscoring the importance of precise allergy assessment to avoid unnecessary alternative prophylaxis.
